As last year, we are distributing sample blank copies of
the Annual Report materials as Adobe Acrobat files. To view
them, you will need to have Acrobat Reader installed. Many PCs
have this utility pre-installed, but if you click on one of
the links below and are unable to read the documents, refer to
the second
half of this page for download and installation
instructions.
You will need one document from each section below.
Click on the link corresponding to your school type:
Section 1:
Section 2: Annual Report Sample. Click on your school
level.
Section 3: Endorsement Section. Click on your school's
endorsement type.
If you have any problems completing your Annual Report,
please call technical support at 800-525-9517. Our technical
support department will attempt to assist you, but may forward
your question to your state office.
